Carrum Health–Surgical Benefit

Carrum Health, Peraton’s Surgical Care Benefit, provides access to top doctors and facilities in the country. Carrum’s specialty networks offer you access to the top 10% of world-class hospitals and surgeons—providers proven to deliver better outcomes and offer services at reduced costs.

To ensure optimal health outcomes, Peraton requires Carrum Health be used for bariatric surgery. Other types of surgeries require an education session with Carrum Health before making provider selection. View the Requirements For Certain Surgeries section below for details.

When you receive care through Carrum Health, we cover all medical costs after you have reached your deductible. Note: the DynamicCare Plan with Surest does not have a deductible. View the Coverage Details section below for additional information.

Requirements for Certain Surgeries

For bariatric surgery, AmeriBen and Surest members must use a Carrum Health Center of Excellence (COE). If you do not use a Carrum COE, your surgery will not be covered.

For spinal fusion or total knee and hip replacements, AmeriBen and Surest members must attend an education session to learn more about Carrum. After the education session, you can choose to have your procedure through Carrum or with another provider. If you do not attend an education session, your surgery will not be covered.

1. Members must meet all or part of their AmeriBen deductible before costs are covered at 100%. The DynamicCare Plan (Surest) does not have a deductible.

2. Travel expenses are covered based on procedure and proximity to a center of excellence, and, per IRS regulations, will be reported as taxable income.
