If you’re diagnosed with a chronic condition, like asthma, arthritis, cancer, heart disease, obesity, osteoporosis, or type 2 diabetes, you want to be sure you do what it takes to manage and improve your condition. Ask yourself these questions as you begin to navigate your benefits:

  • What your diagnosis means for you.

Ask your doctor if you should see a specialist based on your diagnosis. If you do, you can contact Peraton’s Benefits & Beyond Care Advocate, powered by Accolade to help you find a specialist who’s part of the plan’s network.

If you need to take medication, be sure to ask for a generic drug or, if only a brand-name drug is available, check if it’s covered under your prescription drug plan’s formulary drug list to ensure you pay a lower cost.

Peraton provides access to the following programs at no cost to you, if enrolled in a Peraton AmeriBen medical plan, to treat certain chronic conditions:

Virta Diabetes Management, which is a comprehensive type 2 diabetes, prediabetes, and weight management treatment that helps you either reverse or better manage your condition(s), while also providing safe and sustainable weight loss.

Unlike other diabetes (or weight loss) treatments, Virta goes beyond just treating the symptoms of the disease. On Virta, you learn how to change how you eat so that your body burns fat for energy, instead of sugar and carbohydrates. This can help you naturally lower your blood sugar and reduce the need for diabetes medication. It also can help you lose weight and live a healthier life.

Peraton fully covers the cost of Virta (valued at over $3,000) for you and your eligible family members with type 2 diabetes or prediabetes. Virta is also covered for those with a BMI of 30 or greater who are interested in safe and sustainable weight loss.

Hinge Health, which is a digital physical therapy program that offers innovative digital and virtual programs to help manage and treat chronic back, knee, hip, neck, shoulder, and other joint pain through easy-to-access therapy sessions. They can even help you reduce or eliminate medication.

Hinge Health also offers a pelvic health program, which supports women at all stages of life, including pregnancy, postpartum and menopause, when pelvic disorders are most common, and provides care for the full continuum of pelvic disorders.

Accolade Care, which provides extensive support for all acute and chronic conditions. You’ll work with your Care Advocate or a dedicated Nurse Case Manager, to receive personalized care. From condition education and finding a great specialist to helping schedule appointments and coordinating your care, Accolade provides support throughout your healthcare journey.

Carrum’s Surgical Benefit partners with top hospitals, or Centers of Excellence, to give you access to the best doctors and care for your medical needs. You’ll have access to the top 10% of world-class hospitals and surgeons for knee, hip, shoulder, back, neck, weight loss, heart and cancer procedures. With the exception of second opinions, individuals enrolled in a high deductible health plan (Classic or Value Plan) will need to meet the federal minimum deductible as established by the IRS each year, but then all costs will be covered including copays or coinsurance. The federal minimums are typically only a portion of the full plan deductible so using Carrum will likely be financially advantageous. Travel expenses are also covered for you and a companion (if needed). In addition to your dedicated specialist, a mobile app can help you easily prepare, plan and complete paperwork.

Carrum’s Cancer Support provides guidance from experts from leading cancer care centers in the country for any type of cancer. You’ll also receive:

  • ­Ongoing virtual consultations with premier oncologists
  • Access to compassionate nurses who specialize in cancer care
  • Surgery for breast and thyroid cancer
  • Holistic, whole-body supportive care
  • Family and caregiver support
  • Genetic testing for more targeted care
  • Coverage for travel expenses (if needed) for you and a companion

Yes, with Peraton’s 2ndMD program, you can connect with board-certified leading doctors across the country for an expert second opinion via video or phone within days.

If you are unable to work due to a chronic condition, you may be eligible for Short-Term Disability benefits, which continue a percentage of your pay for a certain amount of time; Peraton provides this benefit at no cost to you. If you elected Long-Term Disability coverage, you may be eligible to receive 60% of your pay if your qualifying disability goes beyond 26 weeks.

Peraton provides the Lyra Mental Health Benefit to all benefits-eligible employees, which includes 10 free and confidential mental health coaching or therapy sessions per person per year, along with support with complex needs, like medication management and more.
