WELL-BEING

Preventive Care

Peraton believes preventive care is so important to the health and well-being of our employees. Take advantage of the preventive care coverage provided under the various health plans, so you can be well.

Because Peraton believes preventive care is so important, in-network preventive care—like annual physicals and wellness screenings—is covered 100% under the Anthem, Kaiser, HMSA, Triple S and UnitedHealthcare Global medical plan options. Note that for a service to be considered “preventive care,” your provider must code the service as “preventive.”

Each supplemental health plan pays a lump-sum amount for getting certain preventive care screenings—$50 for Accident Insurance, $50 for Critical Illness Insurance, $50 for the Low Hospital Indemnity Plan, $100 for the Medium Hospital Indemnity Plan and $100 for the High Hospital Indemnity Plan. For more information about the types of screenings that are eligible, refer to the Supplemental Health Plan policy documents on The Lens.

Because Peraton believes preventive care is essential to good dental health, in-network cleanings and oral exams are covered 100% twice a year under the Premier and Value Plans.

Note: Your option’s annual benefit limit applies per participant to all preventive care.

You pay only $10 for an eye exam when you receive care from a VSP provider. Note: Regular eye exams are an important part of detecting eye diseases early and preserving your vision. Early treatment is critically important to prevent some common eye diseases from causing permanent vision loss or blindness.
